We now offer online patient access to the coded data in patient’s health records. This includes allergies, medications, test results, immunisations, problems and codes used in consultations.
Access will be assessed and enabled on a case by case basis. If you would like to request online access to your record please ask reception for a form and information leaflet. Alternatively click on the links below to read the leaflet and print and complete the request form which can then be handed to reception.
In order to receive a registration form and to gain access to your medical record you will need to provide two forms of identification one of which must be photographic.
